We herein present first case a patient during pregnancy and discuss difficulty associated its diagnosis treatment. Case presentation A 39-year-old female at 34 weeks gestation presented sudden left back pain dyspnea. Chest radiography revealed massive pleural effusion. Computed tomography showed bleeding from intercostal artery. Although appeared hemodynamically stable, fetus was critical condition. Emergency caesarean section performed within hour. Subsequently, we endovascular coil embolization While this intensive treatment saved patient, her could not be rescued. Conclusions Patients may develop without gross lesions. In late pregnancy, sufficient infusion quick hemostasis essential can lifesaving.
